8/20/2023 EnchantaVaughn Approved Behavior: Client exhibit a poor attitude when transitioning from normal routine to therapeutic setting. During this time, client was not productive. Client displayed hostile and disruptive manners and needed a small break to get her self together. Client was not in favor of todayâs session, as she continued to give everyone a hard time. Client look lost in her own thoughts and feelings, as she demonstrated a persistent failure to comply to the rules and expectations and respect for QMHP within the session. Client has struggled to ignore distractions and finds her self off task for most of the day. Behavior: client showed unwillingness to engage in the daily activities provided to her during the session. After reviewing ISP, client shows difficulty in following home rules often choosing to act out on purpose. Mom have reported several times that client refuses to listen to her. Client displays outburst when confronted about negative behaviors. Client was reported to have required 2 to 3 prompts prior to being introduced to the home activity.Client was confronted about her current and displeasing behaviors. Client would not own up to anything and continue to blame others for her mistakes. Intervention: QMHP listened to mother verbalize how client became agitated when she didnât get what she wanted when she wanted it. QMHP encourage mother to allow client to express her self in an age appropriate manner and to redirect when needed. QMHP modeled prosocial behaviors for client to strengthen her understanding of them. QMHP intervened to prevent the negative behaviors from escalating. QMHP train client and taking steps to prevent problems including displaying positive home behaviors. Intervention: QMHP refocused client as she was observed becoming distracted. QMHP will review and give feedback regarding a DHD and itâs causes an affect toward client will be an academic functioning. QMHP will provide client with support and encouragement. QMHP will monitor, observe, and redirect client behaviors as needed. QMHP will build rapport.QMHP will explore positive self talk, use instruction modeling, and increase social communication skills with positive reinforcement role-play activity. QMHP use positive regard as she told the client that she appeared to have positive whole management. Response: Client started talking after she became a little receptive QMHP talk to her and coached her on Waze to communicate without being hostile or agitated. Client reported to QMHP that she doesnât like the way that people talk to her in the house and that she was not happy with having to hold back how she really feels, in order to prevent Her self from saying the wrong thing. Client demonstrated more acceptable responses, avoiding anger provoking situation in the home. Client is observed making better choices and doing the right thing. Client has improved with staying in and utilizing her breaks appropriately without needing to display attention seeking behaviors. Response: Client was able to be redirected successfully. Client stated she would try better self management as the day pursues. Client learned a lot from observing others and wanted to prove she can be responsible by participating in all the leisure and fundamental activities and showing good sportsmanship. Client talked about things she loves about being in service and what things she would love to do more now that sheâs limited to mostly inside resources. Client communicated that she feels that she will be better behaved, due to consistent support in the home with mom and seeing QMHP4 times a week. Client took her mind from thinking and doing to a place of concentration and focus for the remainder of the session.
